For discharge planners, case managers and physicians

Send them home to someone who stays.

Adult Foster Care keeps an adult on MassHealth at home with a trained, paid caregiver who lives with them, backed by a nurse and a care manager. One call, and we take the referral from there.

Why Adult Foster Care

A safe discharge, and a home that holds.

For the right patient, AFC is the difference between a nursing facility placement and going home. The caregiver is already there. We make the arrangement paid, trained and clinically supervised under MassHealth regulation 130 CMR 408.000.

Care that lives in the home

A family member or friend, not a spouse, who lives with the member gives daily hands on help with personal care, every day, not in shifts.

A clinical team that visits

A registered nurse and a care manager take turns visiting the home, six visits each a year, so someone from the team is there every month.

On call around the clock

Caregivers can reach our team 24 hours a day, seven days a week, so a question at night does not have to become a trip to the emergency room.

What happens after you refer

You make one call. We carry the rest.

  1. 01

We call back within four business hours

A real person from our intake team, not a voicemail queue, confirms what you sent and agrees the next step with you.

  2. 02

We screen and contact the family

We confirm MassHealth coverage, the need for daily help and the living arrangement, in the family's preferred language.

  3. 03

    We handle the paperwork

Clinical assessment, the physician's form, caregiver screening and training, and the MassHealth authorization. We keep you posted as it moves.

A good referral

Who to send our way.

You do not need to confirm eligibility before you call. If you are unsure, refer, and we will check.

  • An adult on MassHealth

Age 16 or older, with active MassHealth coverage or an application in progress.

  • Needs daily help with personal care

Bathing, dressing, eating, toileting, moving safely or taking medications, every day.

  • Someone lives with them, or could

A relative or friend in the same Massachusetts home who is willing to be the caregiver. Not a spouse.

  • Wants to stay at home

The member and family prefer home over a facility, and the home can be made safe for care.
